STEP 1

Print and complete the Mid-Atlantic Multisport Athlete Questionnaire. This questionnaire provides Mid-Atlantic Multisport with details on your goals, background, current fitness level and time constraints and allows your coach to create a training program that is personally tailored to your needs. STEP 2

Fax the completed questionnaire to Mid-Atlantic Multisport at
610 -788 -2105.  You may also mail it to:

STEP 3

Have a one-on-one personal consultation with your coach to discuss your individualized training program.

STEP 4

PREPARE TO SUCCEED!!!!
